The product is not ABA-specific, but its drag-and-drop builder, large template library, conditional logic, payment fields, file uploads, and e-signature are comprehensive enough that many agencies adopt it as the intake forms tool that sits alongside their clinical platform.

The company has been building forms software since 2006 and has grown into one of the largest online forms providers globally, with a free tier, multiple paid tiers, and HIPAA compliance available on higher plans.

Who it's for

Jotform's clearest fit for ABA is the solo BCBA or small clinic that needs a professional intake form fast. Setup time is short — a working form can be built and embedded on a website in under an hour — and the price point is accessible. It is also a common pairing for agencies whose clinical platform has weak forms; rather than fight the clinical tool, the agency runs intake in Jotform and pulls signed paperwork into the clinical record manually.

It is less useful for agencies that want a single ABA-specific workspace. Jotform models forms; it does not model intake workflows, client records, family portals, or recurring task creation. For an agency with 30+ active clients, the disconnect between Jotform and the clinical platform becomes meaningful operational drag.

What stands out

The form builder is the standout. Conditional logic is mature, the template library is broad, and the design surface is flexible enough that an agency can ship a form that looks polished without paying a designer. E-signature is built in, file uploads are handled, and payment fields work without a separate tool.

Where it falls short

Jotform is not ABA-specific. Templates and terminology assume a generic forms use case, which means real customization is required to match an ABA intake workflow. Operators report meaningful one-time setup work translating Jotform into a fit for ABA.

HIPAA compliance is gated to higher plan tiers. Smaller agencies on the Bronze plan who assume HIPAA is included sometimes get an unwelcome surprise when they realize they need to upgrade. Confirm the plan tier covers HIPAA for any form that touches protected health information.

The branded experience is limited. Custom logos, colors, and domains are supported, but the underlying UI still reads as Jotform — parents do not feel like they are inside the agency.
