For ABA agencies, the most common reason to pick eFax over alternatives is brand recognition: payers, vendors, and counterparties have all heard of eFax, which reduces friction in workflows where the recipient verifies the sender's fax provider.
Who it's for
eFax's clearest fit is the mid-to-large ABA agency that values the most universally recognized fax brand for compliance and counterparty acceptance. Agencies with existing eFax usage often stick with it simply because the switching cost (number portability, training staff, updating payer records) outweighs the savings of a cheaper alternative.

Where it falls short
Pricing is the most consistent operator complaint. eFax costs more than SRFax and most direct competitors at comparable feature levels. For solo BCBAs and small clinics, the premium is hard to justify.
HIPAA tier gating is a related issue. The entry-level eFax Plus plan does not include HIPAA-eligible BAA — that requires eFax Corporate or specific HIPAA-tier products. Operators starting on the cheaper plan are sometimes surprised when they find compliance requires an upgrade.
Customer service quality is variable. The large legacy account base means small customers can feel deprioritized, and the upsell-heavy account management can frustrate operators who want a simple fax service.
